CONTESTANT APPLICATION INSTRUCTIONS
1. Please print this application and fill it out legibly, using dark colored ink.
2. Answer all questions honestly and to the best of your ability.
3. Write on the printed side of paper only, but feel free to attach additional sheets of paper if necessary.
4. E-mail a digital photo of yourself when you submit this application.
